Description
The General Surgery Residency Program at Parkview Regional Medical Center, Fort Wayne, IN has an ACGME accredited CATEGORICAL training position available at the PGY2 level beginning JULY 1, 2026. Please see the specific requirements and instructions for applying below. NOTE: It is the policy of Parkview Health Graduate Medical Education to sponsor J-1 Visas only.
PREREQUISITES (no exceptions):
- Successful completion in good standing of ONE year of US clinical training in an ACGME accredited general surgery residency training program.
- Must have graduated from an Accredited Medical School or
- International Medical Graduates must have a valid ECFMG certificate (Educational Commission for Foreign Medical Graduates)

- Brief Personal Statement
- Medical School Diploma and Transcript
- Dean's Letter
- USMLE Parts I, II, III scores (must be first pass) or COMLEX I, II-CE, III scores
- ABSITE scores and reports for ALL years completed
- Three (3) current Letters of Recommendation. Including one from the current Program Director will be required.
- Rotation schedule with Competency-Based Evaluations for ALL completed rotations
- Current year semi-annual Evaluation
- Current ACGME “General Surgery Defined Category and Minimum” and “General Surgery Year 2 Minimum” case log reports
- ACLS, BLS and ATLS certification
- FLS and FES Certifications (if available)
- Copy of ECFMG (if applicable)
- Copy of current DS2019 for J-1 Visa holders (if applicable)
